To schedule an appointment with Newbridge Clinic, complete the following steps:

(1) For new patient appointments, email appt@newbridgeclinic.com, or call us to receive a copy of our new patient intake forms including our fee schedule / insurance information, and to schedule an appointment.  For follow-up appointments, call our main number at 612-730-2237 to schedule an appointment.

(2) Complete intake forms and return them to us at least 1 week prior to your scheduled appointment.  Follow-up appointment forms should be received at least 24 hours (1 business day) prior to your appointment.

(3) E-mail (or if needed, mail or fax) them to:
